Medication at school
For the safety of all students, all medications must be administered through the Nurse’s Office.
This includes:
- Prescription Medications
- Over-the-counter medications
- Vitamins and supplements
- Cough drops and medicated creams
If your child requires medication during the school day, please have your healthcare provider complete a Medication Authorization Form.
Please submit the following to the Nurse’s Office:
- A completed Medication Authorization Form
- Medication in its original, properly labeled container
- Medication clearly labeled with the student's name and date of birth
- Medication placed in a Ziploc bag for delivery to the clinic
Please do not send medication to school with your child or place it in their backpack. A parent, guardian, or designated adult must deliver all medications directly to the Nurse’s Office in accordance with Deer Park ISD Policy.
Allergies and Medical Conditions
If your child has a health condition that may require special care or emergency intervention during the school day, please provide the Nurse’s Office with an updated Emergency Action Plan completed by your child’s healthcare provider.
This includes, but is not limited to the following:
- Asthma
- Seizure disorders
- Diabetes
- Food allergies
- Severe insect sting allergies
- Other medical conditions requiring special attention or emergency care
Completed Emergency Action Plans along with any required emergency medications, such as inhalers, epinephrine auto-injectors (EpiPens), seizure rescue medications, or diabetic supplies.
All emergency medications must be properly labeled with the student’s name and date of birth.
Having current medical documentation on file helps us respond quickly and appropriately should an emergency arise.
When to keep your child home
To help prevent the spread of illness, please keep your child home if they have:
- A fever of 100°F or higher
- Vomiting or diarrhea
- Uncontrolled coughing
- Any contagious illness as determined by a healthcare provider.
Students should be fever-free for a minimum of 24 hours without fever-reducing medication before returning to school.
Communication with the Nurse’s Office
Please notify the Nurse’s Office if there are any changes to your child’s:
- Medical condition
- Allergies
- Medications
- Emergency contact information
